2010-2011 Immunization Information
Many students participated in our in school clinic to bring them up to date with next years new immunization requirements. For those who have not, the Monday evening clinic is available at Perry County Family Practice from 2:30pm-5:30pm, and from 5:30 to 6pm for appointment only. They can be reached at 547-2746 to make appointments. The requirements are as follows:
All 6 through 12th graders are required to have a TDaP(within the last 5 years), an MCV4, and a 2nd Varicella or documentation of the chickenpox disease. Parents need to provide us with a date that the student had the chicken pox and this must be documented on the health record. If this information cannot be provided then the student needs the vaccine.
Cost for the vaccines at the clinic are $8.00/shot. There is a Tuesday clinic in Cannelton at the annex building from 9-11am and 1-4pm that can also be attended to obtain vaccines. This clinic is also provided by the Perry County Health Department. Students need to be up to date by the start of next school.
